Form & Fitness was founded in 2001 by physical therapy and fitness professional Dr. Ben Quist. The focus has always has been on world-class personal training in a private facility were the best personal training and physical therapy professional in the area could come together to practice their craft. This blend of fitness and rehab has quickly become and still is a premier personal training facility in southeast Wisconsin.
