Katie Dykas

About me
I am a personal trainer at the Grafton Form and Fitness location. I work with the Parisi Speed School athletes as a strength and speed coach as well. Fitness has been a part of my life since I was little, so it was only a short time to figure out what I wanted to do with my future. In high school I was a two sport athlete; volleyball and basketball. I then went on to play basketball at Lakeland college, but soon transferred to University of Wisconsin-Oshkosh. I gain the motivation by watching my clients achieve their goals and get the results they truly deserve. I believe in designing safe, fun, and effective workouts that will guarantee results.

Annie Farley

About me
As a two-sport collegiate athlete, I developed a passion for strength and conditioning, and coaching.  As an undergraduate, I studied biology and exercise science, culminating in post-graduate studies in exercise physiology.  Sharing my knowledge, and the enjoyment of being active has been the focus of my 12 year personal training journey. Adding a Precision Nutrition certification has given me an additional tool to help my clients achieve their goals.

I love to teach group exercise classes. Indoor cycling, strength training, as well as sports-specific conditioning are among my favorite. Away from the club, I enjoy tennis, skiing, and riding horses. My husband, Mike and I live in Cedarburg, and are the parents of 4 great kids and two Goldendoodles.

Polly Hackett

About me
I have been a certified personal trainer for over 12 years, however, sports and fitness has been
my passion my entire life. As a young child I was involved in skiing, soft ball, tennis, and then
cheerleading in high school.
I was diagnosed with Type 1 diabetes my senior year in college and insulin dependent since
that time, which kicked my focus on fitness to an even higher level. I have run marathons, 1/2
marathons, and trail runs across the country.
I moved to Mequon a couple of years ago with my family and have enjoyed working with people
looking to create an overall fitness level that allows them to live a lifestyle of activity and fun. I
specialize in different areas of fitness, working with my clients on weight loss, achieving fitness goals both large and small. My goal is to introduce my clients to a lifestyle of fitness not just a 3 month
experiment. It has to be fun and it has to show results, that is what makes my job so much fun!

Jonathan Grodzki

About me:
I have spent most of my life playing hockey and running track. I received my NASM CPT during school and started training in 2018. I graduated from Concordia University with a Bachelor of Science in Exercise Physiology and am currently pursuing my master’s in Kinesiology. My favorite food must be ice cream. Currently my workouts kind of change all the time but if I had to pick one, I would choose Olympic lifting. There is always something to improve upon. The best song for me would be general by dispatch.

Fun Fact:
I like to build computers and work on cars whenever I am not working out.

Degree/Certification:

Concordia University-Bachelor of Science in Exercise Physiology
NASM Certified Personal Trainer

Abi Marcquenski

About me:
I have been in a health and fitness setting for most of my life. Playing 3 sports through high school, up to college, and my parents running their own fitness facility for over 10 years, it was almost inevitable that I would become passionate about making a career out of the exercise science field. That being said, I have had very rewarding experiences working with the general population who want to shed a few pounds, improve their overall quality of life, move around pain free again, or improve their physique, as well as, working with athletes who want to become more successful in their sport.

Throughout my years in the gym and working with other fitness professionals, I have found and learned that consistency, self discipline, and balance is THE secret to reaching and sustaining any and all goals. My job will be to broaden your knowledge, help you stay consistent, remain injury free, but most importantly, to ENJOY THE PROCESS.

Certifications/Degree:

Bachelors in Exercise Science at the University of Wisconsin Parkside

Mike L.

I joined Form & Fitness to find the person I once was… I tell people that it’s not easy, but then again, it’s not too hard. I know have a wife who takes time to stop and checks me out!

Becky H.

Within eight weeks of joining Form & Fitness, I lost a healthy seven pounds, which was great motivation for me to keep going!

Connie G.

I feel so much better when I’m done working out.  It’s a big stress reliever, which makes me want to come back.
